Transcript Request

To request transcripts, please:

  1. Download, complete, and sign the Transcript Request Form.
  2. Fax or mail the completed form to the attention of the Registrar.
  3. Send $5 payment for each transcript requested. The payment may be sent by a check payable to Westminster Academy or in cash. Checks should be mailed to the attention of the Registrar. The fee is waived for current students and for students who graduated or withdrew during the previous school year.
  4. Once authorization to release the records has been received from the Finance Office and the payment has been received, then the records will be sent to the place(s) indicated on the Transcript Request Form
